Extending the Lifespan of the Penile Prosthesis

Penile prostheses are used when men experience problems with achieving or maintaining a firm and sustainable erection for sexual intercourse. These devices enhance erection by providing sufficient support to the penis to achieve optimal rigidity for a satisfying sexual relationship.

: There are two main types of penile prosthesis

1. Malleable Penile Prosthesis: It consists of two flexible tubes of smooth material made of the finest polyurethane or medical silicone, which are surgically implanted in the penis through a small opening in the scrotum. A man can bend the penis down throughout the day so it is not visible through clothing and then bend it up to achieve an erection right before intercourse.

2. Inflatable Penile Prosthesis: It is considered the most widespread and technologically advanced over the malleable prosthesis. If the malleable penile prosthesis provides the man with an erection and then a downward bend, then the inflatable penile prosthesis provides him with an erection followed by complete relaxation similar to the natural relaxation of the penis.

An inflatable prosthesis consists of two surgically implanted hydraulic cylinders inside the penis, through a small incision in the scrotal sac, which is closed cosmetically without leaving any trace. There is a reservoir implanted under the abdominal muscles that contains fluid and remains invisible, and a pump implanted in the scrotal sac to operate the prostheses when needed before intercourse.
Thus, the inflatable prosthesis allows the man to control the timing, strength, and duration of the erection and provides him with a more natural experience that is closest to a natural erection.

A man should consult Professor Dr. Adham Zaazaa, Professor of Andrology at the Andrology Department at Cairo Unversity, to determine the appropriate type of erectile prosthesis according to his condition and medical needs.

Factors that prolong the lifespan of the penile prosthesis:

Penile prosthesis implantation must be performed by an integrated medical team specializing in this type of delicate surgery. The surgeon must have long experience and have a record of the largest possible number of penile prosthesis surgeries to ensure safe and effective results.

Professor Dr. Adham Zaazaa has over twenty years of experience in penile prosthesis implantation. He began choosing his supporting specialized medical team in 2001 to assist him in this precise specialty and is keen to apply all the international scientific standards necessary to prolong the lifespan of the erectile prosthesis, which ranges from 18 to 25 years. The most important of these factors are the following:
1. Choosing the type of prosthesis: It is important to select the best type of prosthesis, as higher quality prostheses are associated with fewer potential issues. The benefits of an extended lifespan outweigh the long-term costs even if the cost of the best prosthesis may be higher.
2. Applying modern techniques: The techniques used during the procedure have a significant impact on extending the expected lifespan of the penile prosthesis. Professor Dr. Adham Zaazaa has developed and published several advanced implantation techniques under his name. “Zaazaa Advanced Implantation Techniques”. Many of these techniques were developed by Dr. Adham himself and published under his name in reputable scientific references and journals related to penile prosthesis implantation.

Some of Professor Adham Zaazaa’s techniques:

Zaazaa Advanced Implantation Techniques are innovative techniques that have been globally published under Professor Zaazaa’s name.

Zaazaa Caversonal Sparing Technique:
This technique for preserving the cavernous tissues and arteries ensures the best shape, texture, and temperature of the penis after the procedure and ensures the safety of blood circulation to the penis around the prosthesis. This maintains the natural tumescence of the penis during arousal and intercourse. It also ensures that antibiotics reach the prosthesis after the procedure to protect it from bacteria, which explains the high success rates achieved by Professor Dr. Adham Zazaa in implanting penile implants, to the point that he was awarded the Leadership Award in penile prosthesis implantation procedures for achieving the highest success rates in implanting implants in Egypt and the Arab world.
In addition, the prosthesis implantation is performed while preserving the cavernosal tissue, ensuring that the prosthesis is surrounded by the natural cavernosal tissue from all sides. This is one of the unique techniques developed and improved by Professor Dr. Adham Zaazaa, a pioneer in male prosthesis surgery in Egypt and the Middle East.

Zaazaa Pre-Set Monofilament Needle-less Air Sutures:
This technique involves using pre-prepared air sutures resistant to bacteria to ensure complete safety during the prosthesis implantation procedure. Without these pre-prepared sutures, some surgeons may inadvertently scratch or puncture the outer surface of the prosthesis, which can allow bacteria, blood, or other fluids to enter and potentially reduce its expected lifespan. Therefore, applying the pre-set monofilament needle-less air sutures technique eliminates the possibility of scratching the outer wall of the prosthesis, enhancing the quality, efficiency, and expected lifespan of the device.

Zaazaa TSMT “Tumescent Stretching Measurement Technique” and PIIMP Charting
Zaazaa techniques for measurement are precise surgical techniques used during the procedure to ensure maximum length and girth of the penis, guaranteeing the highest levels of satisfaction for couples after the surgery. This is achieved by implanting the prosthesis in the erect penis during the procedure. Artificial erection is induced to measure the ideal depth, thickness, and length of the erect penis, accurately determining the appropriate length of the prosthesis to be implanted. Shortening the prosthesis exposes the penis to bending or folding during intercourse, potentially reducing its expected lifespan. Similarly, using a prosthesis longer than necessary strains the surrounding tissues and reduces its expected lifespan.

No-Touch Technique:
The No-Touch Technique, applied by Professor Dr. Adham Zaazaa, enables him to implant the prosthesis inside the penis without touching the prosthesis to any surface during the procedure or the patient’s skin, ensuring that bacteria will not transfer from the patient’s skin to the surface of the prosthesis, as these bacteria could potentially reduce the prosthesis’s expected lifespan.
