I regained Erection after a Prostatectomy with a Penile Prosthesis
My name is Omar, and I am sixty-four years old. I experienced erectile dysfunction. My story began with the discovery of a tumor in my prostate during a routine check-up, which necessitated a prostate removal surgery.

After the prostatectomy, my erections started to weaken. I tried various types of medications, but unfortunately, I didn’t get any results. Therefore, I went to Dr. Adham Zaazaa, a consultant in male sexual disorders and ICSI and a professor of andrology and fertility at Cairo University. After the examination and tests, he advised me to undergo penile prosthesis implantation, informing me that it has helped many men who underwent prostatectomy and experienced erectile dysfunction like me.

Why does prostatectomy cause erectile dysfunction?

Removal of the prostate, whether partial or total, may lead to erectile dysfunction for the following reasons:

1. Nerve damage: During the prostatectomy procedure, the nerves surrounding the prostate gland can be damaged. These nerves play a crucial role in achieving an erection. If these nerves are damaged or exposed to cut, the body may struggle to achieve or maintain a strong erection.
2. Vascular disorders: Prostatectomy can cause changes in blood circulation that affect the blood vessels in the genital area, resulting in reduced blood flow to the penis. This can affect the ability to achieve and maintain an erection.
3. Psychological factors: Prostatectomy can have psychological effects on men, such as anxiety or depression, which can impact sexual performance and lead to erectile dysfunction.

Erectile dysfunction unresponsive to medications:

If penile tissues start to atrophy, the patient may notice a gradual shortening of the penis. This type of erectile dysfunction may not respond to medication. However, surgical intervention, specifically penile prosthesis implantation using the latest global techniques registered under the name of Professor Adham Zaazaa, can contribute to the restoration of firm and sufficient erections by preserving the cavernous tissues of the penis.

Penile Prosthesis after Prostatectomy:

Penile prostheses are usually implanted when other treatments, like in the case of Mr. Omar, have failed to address erectile dysfunction. They represent an optimal solution for cases of erectile weakness, and the implantation procedure typically takes no more than ninety minutes.

There are two main types of penile prostheses:

1. Inflatable penile prostheses: consist of two cylinders, a pump, and a small, saline-filled reservoir. The surgeon places the cylinders in the penis, the pump in the scrotum, and a small saline reservoir under the abdominal muscles.
2. Malleable penile prostheses: consist of two flexible rods that the surgeon implants to achieve a strong erection for sexual intercourse. These prostheses are made of advanced medical materials that do not interact with the body, allowing for changes in the position of the penis as needed. The patient can adjust the position of the penis upwards during intercourse or downwards throughout the day or during urination.

Inflatable penile prostheses are the most advanced option for penile implantation. With a penile prosthesis, erections can be maintained as long as desired, as the prosthesis can be controlled.

Sexual intercourse can be resumed four to six weeks after penile prosthesis implantation.

These prostheses provide sufficient rigidity for men with severe erectile dysfunction to engage in sexual activity. They restore the natural ability to penetrate, achieve orgasm, ejaculate, and achieve sexual satisfaction for both partners.

After the procedure, I felt rejuvenated, and my nervousness and moodiness disappeared. I would like to express my gratitude to Dr. Adham Zaazaa, a pioneer in male penile prosthesis surgery in Egypt and the Middle East. His extensive experience in the field of male sexual medicine has enabled him to assist in numerous cases of various sexual dysfunctions.
